Self Certification Form - This covers you for your first 7 days of absence from work. If you need longer than 7 days off work you can contact us on 01302874551 or via Accurx online for a signed fit note after completing a self certification.

Podiatry Referral Form for Cantley Health Centre - Once this form is completed, you are able to self refer to the Cantley Health Centre by sending your referral form to the following email address, rdash.podiatryreferrals@nhs.net. Alternativly, please mail and address your referral form to Podiatry Services, Cantley Health Centre, Middleham Road, Goodison Boulevard, Doncaster DN4 6ED.

Access to your Online Medical Records Form - Please complete this form appropriately and return to a branch of your choice along with photo identification and documentation of your home address. Alternatively please email your completed form and ID documents to syicb-doncaster.dvh@nhs.net. 

Proxy Access to Online Medical Records - A GP surgery can give someone proxy access so they can help another person manage their care. If you are a parent wanting to gain proxy access to your child's medical record, please complete the above form and provide a copy of their birth certificate alongside your own photo ID. If you are needing proxy access for a consenting adult over the age of 18, please have them sign the appropriate parts of the above form and return to the surgery with photo ID or power of attorney documentation.

Travel Risk Assessment Form - If you are travelling to a foreign country for business or leisure, you may need vaccinations prior to your departure. Please return the completed form to a DVH branch in person or via syicb-doncaster.dvh@nhs.net and we will book a suitable telephone consultation with a practice nurse.
